Agreement to these terms
By using the Axistify app or website you agree to these terms. If you do not agree, do not use the service. Axistify is operated by IconicTek, LLC.
Axistify is not a medical service
Axistify does not provide medical advice, diagnosis or treatment. It is a communication and monitoring tool that carries information between you and the clinicians responsible for your care. Clinical decisions are made by those clinicians, not by the app.
Do not use Axistify in an emergency. If you think you are having a medical emergency, call your local emergency number. Messages and readings are not monitored continuously, and no one is guaranteed to see them immediately.

Accounts
Accounts are issued by your healthcare provider. You cannot register yourself, and you may not share your account or your sign-in details with anyone. Tell your provider immediately if you believe someone else has access to your account.
Your provider can suspend or end your access at any time, for example when you are discharged from their care.
Using the service
You agree not to:
- use the service for anything other than your own care;
- attempt to access information belonging to other patients or organisations;
- interfere with the service, probe it for weaknesses, or work around its access controls;
- upload content that is unlawful, or that you do not have the right to share;
- record a video visit without the agreement of everyone taking part.
Monitoring devices
Bluetooth monitoring devices are supplied or approved by your provider. Follow the manufacturer's instructions when taking a reading. Axistify displays and transmits what a device reports — it does not verify that a device is accurate, calibrated, or used correctly. If a reading looks wrong, take it again and tell your care team.
Video visits
Video visits are scheduled by your care team and take place inside the app. They depend on your internet connection and your device, neither of which is under our control. If a visit cannot go ahead, contact your provider to rearrange it.
Exercise programmes
Exercise programmes are prescribed by your clinician, not by Axistify. Follow what your clinician has told you, stop if something is painful, and tell them. The pain and effort ratings in the app are optional and are shared with your care team.
Your content
You keep ownership of what you put into Axistify — your messages, photos and readings. You give us permission to store and transmit that content only so we can operate the service for you and your provider.
Availability
We aim to keep Axistify available, but we do not guarantee uninterrupted service. We may take it offline for maintenance, and parts of it depend on services outside our control. [confirm: add any uptime commitment made in provider contracts]
Fees
Patients are not charged through the app. Fees are agreed between Axistify and your healthcare provider.
Liability

Changes
We may update these terms. When we do we will change the date at the top of this page, and where the change is significant we will tell your provider so they can inform you.
